Patho-histologic Study to the Liver in Extrahepatic Biliary Obstruction

Abstract
A series of 25 cases of patient with extrahepatic biliary obstruction, which was confirmed by operation, has reviewed on the patho-histologic change of the liver tissue in comparison with the clinical and laboratory findings.
The results obtained are as follows:
1) The most striking feature of the liver was cholestasis, which resulted in the dilatation of ductules and the bile plugs in canaliculi with a preponderance in the centrilobular area.
There was, in addition, a correlation between the degree of cholestasis and the direct reacting serum bilirubin.
2) There were also a significant degree of ductal cell proliferation, and diffuse precipitation of bile pigmented granules both in liver cells and in Kupffer cells.
3) Ballooning and feathery degeneration of liver cells were observed in 17 cases(68%), and focal recrosis of isolated liver cells of the centrilobular area were found in 20 cases, which showed a marked increase of S-GPT.
4) Edema of the portal tract, proliferation of collagen fiber, periductal fibrosis and infiltration of inflammatorycells were also significantly observed.
5) Of 25 cases, 6 (24%) showed an evident cirrhotic pattern, with a correlation to duration and the degree of the obstruction.
